Occupational Therapist - Community Aged Care

Our Hobart office is a multidisciplinary team of OTs, Speechies, Physios, EPs and BSPs, supporting clients across the NDIS and Support at Home programs.
What You’ll Do
Based at our Hobart office, you willprovide OT services to Support at Home clientsin the community.
Support at Homeenablesin-homeagedcare clientsto remain safe, independentand engaged within their own homes.
Your responsibilities will include
- Conducting functional assessments and documenting client needs in detailed reports.
- Communicating identified needs with clients, families, and service providers, and coordinatingappropriate referralsto relevant health professionals or specialists.
- Prescribing suitable equipment and recommending home modifications to support client independence and safety.
