- Working collaboratively with clients, carers, hospital teams and community service providers.
- Brokering and overseeing approved clinical and in-home support services.
- Monitoring and reviewing care plans and supporting transition to longer-term community services where required.
- Advocating for clients and contributing to discharge planning, case conferences and family meetings.
- Maintaining accurate documentation and supporting program reporting, KPI and VINAH requirements.
- Contributing to multidisciplinary practice and quality improvement activities.
About you
You’ll be an experienced allied health professional or nurse with strong clinical care coordination skills and the ability to work collaboratively across hospital and community settings.
To be successful, you’ll bring:
- A relevant tertiary qualification in allied health or nursing, with current registration or eligibility under the relevant professional body.
- A minimum of three years' experience working in hospital and/or community service settings.
- Demonstrated experience in clinical care coordination within acute and sub-acute settings.
- Experience supporting people with complex needs.
- Strong communication and interdisciplinary collaboration skills.
- A client-centred and trauma-informed approach.
- Strong organisational, problem-solving and time-management skills.
