BCR Communities, a not-for-profit community organisation, invites applications for a Care Partner to deliver person-centred home care case management within the Illawarra and Shoalhaven region. The role is full time, based mainly in South Nowra with a hybrid option and requires travel to participants' homes and community settings.
You will work in a multidisciplinary team to develop wellness goals and care plans, aligning with the Support at Home program and Aged Care Quality Standards.
