The Role:
We are seeking a proactive and organised Care Coordinator to join our dynamic team. This is a hands‑on coordination role responsible for ensuring participant supports are delivered safely, consistently and in line with NDIS standards.
Key Responsibilities:
- Coordinate SIL clusters and assigned community participants.
- Actively engage with Participants and respond to changes in service needs and communicate changes with support workers.
- Ensure support plans and documentation remain current.
- Balance participant support needs with workforce sustainability.
- Work closely with rostering team to ensure shifts align with participant needs.
- Coordinate diaries and manage scheduling commitments.
- Liaise with families, allied health professionals and stakeholders.

About You:
- Experienced in disability or community services (desirable, but not essential).
- Proficient in Microsoft Office Suite and data entry.
- Current driver's license and access to a personal vehicle.
- Calm under pressure in a 24/7 support environment.
- Comfortable managing complex, high‑intensity support environments.
- NDIS Worker Screening Check or willingness to obtain one.
- Full working rights in Australia.
