Aveo is seeking a Care Partner (Clinical) to support residents in Aveo retirement communities. You will conduct home care visits, assess needs, and enrol services to help residents live independently longer.
You'll build relationships with residents and families, coordinate care, and document plans using Alayacare. As a Registered Nurse with home care experience, you'll work in a collaborative team, manage various care options, and ensure high-quality, compassionate service for our residents
