Aveo is seeking a Care Partner (Clinical) to join our home care team. As a Registered Nurse, you will be an advocate and trusted partner to residents, delivering personalised home care plans and coordinating services within Aveo communities.
You will meet with residents and families, explain funding options, document care, and support independence with My Aged Care pathways, while using Alayacare and Microsoft Office on mobile devices.
