Australian Vietnamese Women's Association (AVWA) in Melbourne seeks a Care Partner to deliver care management under the Support at Home Program. You will coordinate person-centered plans to enable older people to live safely at home, with a focus on wellness, reablement and culturally safe care.
You will collaborate with clinicians, GPs and carers, develop budgets, monitor risks, and ensure timely care planning and documentation.
