Australian Unity is seeking a Clinical Care Partner in the Northern Rivers region (Ballina/Lismore/Casino) to lead clinical guidance and escalate care within the Neighbourhood Care Partnering model.
You will support customers with high complexity, identify clinical risk, and work with Care Partners and the multidisciplinary team to improve outcomes and ensure timely, appropriate care. This is a Permanent Full-Time role with flexible hours and hybrid work.
