Australian Unity Limited is seeking a Care Partner to support customers on their care journey, developing holistic plans and coordinating services to enable safe, independent living. The role is based at the Hornsby branch with a hybrid work arrangement, combining client visits, branch days and work-from-home flexibility.
You will manage a customer portfolio, perform onboarding assessments, and work with clinical teams, families and external providers to deliver person-centred care and optimal outcomes.
