Ausnew home Care is seeking a suitably qualified and experienced Community Worker to join our team on a full-time basis.
The role involves providing structured support services to individuals and families, assisting them to access appropriate community resources and services in line with assessed needs.
- Key Duties and Responsibilities
- Assessclient needs and provide tailored support to access health care, housing, employment, education, and social services.
- Interview clients to identify social, emotional, financial, and personal challenges and develop appropriate support plans.
- Plan, develop, and deliver community education, training, and support programs to promote wellbeing and independence.
- Work collaboratively with families, communityorganizations, health professionals, and government agencies to provide integrated, client-centeredservices.
- Refer clients to relevant service providers and ensure follow-up to support continuity of care.
- Monitor client progress and evaluate the effectiveness of programs and interventions, recommending improvements where required.
- Maintain accurate and confidential client records, case notes, and reports in line withorganizationaland regulatory requirements.
- Promote social inclusion, equity, and respect for cultural and individual diversity.
- Advocate for clients’ rights and support their participation within community and service systems.
- Support clients in developing life skills, building independence, and engaging with the community.
- Selection Criteria
- A relevant qualification in Community Services, Social Work, or a related discipline, orequivalent relevant experience
- Demonstrated experience in a community services or support role
- Sound communication, interpersonal, andorganizationalskills
- Ability to work both independently and as part of a team
- Knowledge of community support systems and service referral pathways
