Allen Care Services is seeking a Client Care Coordinator to deliver high-quality, person-centered supports for NDIS participants in the Toowoomba region. You will work with participants, families, support workers and allied health professionals to ensure safe, consistent services aligned with each participant's goals and preferences.
Reporting to the Operations Manager, you will oversee intake, support planning, service coordination, risk management and records management, building strong relationships.
