Align Care seeks a dedicated coordinator to assist participants with understanding and implementing their NDIS plans.
You will coordinate services and connect participants with appropriate providers to support independence and goal achievement.
You will liaise with participants, families, carers, the NDIA and other stakeholders to ensure quality outcomes and timely reviews, maintaining accurate case notes and reports throughout.
