You will assess function and falls risk, prescribe and progress exercises to help older people regain strength, balance and independence at home, and work as part of a multidisciplinary team.
You will lead goal‐based restorative and reablement programmes under the Support at Home pathway, coach clients and families, and maintain clear notes.
