If you enjoy helping people feel confident, supported, and in control of their lives - this could be a really great fit.

We're looking for someone who genuinely cares about people, communicates well, and wants a role where they can make a real difference (without burnout or unrealistic expectations).

Whether you've been a Care Manager, Case Manager, or Coordinator, if you understand Home Care Packages, particularly Self Managed, and like building relationships - we'd love to hear from you.

What makes this role different

  • A supportive, easy-going team that actually helps each other out
  • Hybrid setup so you get balance between home and office
  • A role focused on guiding and supporting - not rushing through visits
  • Real career growth as the team continues to expand
  • Extra perks like your birthday off + a YOLO Day (yes, really)

What you'll be doing

  • Supporting older Australians to stay independent at home
  • Working with customers and families to build simple, practical care plans
  • Helping people understand and use their Home Care Package funding
  • Running Telehealth check-ins, reviews, and support callsKeeping things organised, compliant, and running smoothly behind the scenes

What we're looking for

  • Around 1+ year experience in aged care (Care Manager, Case Manager, Coordinator or similar)
  • A good understanding of Home Care Packages
  • Comfortable using systems and typing while talking on calls
  • Someone calm, organised, and easy to talk to
  • Most importantly - someone who genuinely cares about helping people

Why this role matters

This isn't just admin or ticking boxes - you'll be helping people stay in their homes, maintain independence, and feel more in control of their lives.

It's meaningful work, with a team that actually values what you do.
