The role focuses on person-centred planning, psychosocial assessment, risk-responsive coordination, and the delivery of ethically grounded support coordination that promotes safety, participation, and continuity of care.
Key responsibilities
- Work with participants, families/carers, and multidisciplinary providers to implement NDIS plans
- Coordinate funded supports
- Address barriers to access
- Strengthen functional and social outcomes
- Apply advanced clinical judgement and systems-based coordination where complexity requires it
